PROBLEM TO BE SOLVED : To mount all light receiving/emitting elements and optical elements on a single substrate by means of a passive alignment technique by disposing a plurality of optical elements and light receiving/emitting elements on a mounting substrate, and further dividing the mounting substrate into a light source mounting surface and an optical element mounting substrate. SOLUTION : A signal light is made incident from an input port 24. A lens emitted light is collimated, and, at first, it is made incident on a WDM filter 23 board and transmitted through it. Pumping light emitted from an pumping light source is reflected by the WDM filter 23, and made incident on a rare earth doped fiber. The signal light transmitted through the WDM filter 23 is made incident on a light isolator element, and transmitted through it. Then it is made incident on a branching element, and a part of the output light is reflected on an incident end surface, and transmitted through it. A part of the light reflected is made incident on a photoelectric output light monitoring PD, and converted. And it is made incident on an output side fiber. And a reflected return light from the output side fiber is reflected on an output side reflective surface of the branching element, too.
