PROBLEM TO BE SOLVED : To make a user of an accounting slip, when it is outputted a plurality of times, easily and visually recognize that the accounting slip is the re-output one by outputting the accounting slip while automatically adding information showing the re-output thereto. SOLUTION : The form management system (output history management device) 1 comprises : a storage means for storing accounting journal slip information and output history information of accounting slips; a re-output determination means 23 for determining, in response to an output instruction of accounting slip, whether an accounting slip instructed to be output was outputted in the past or not in reference to the output history information; an accounting slip output means (output execution means) 24 for outputting, when it is determined that the accounting slip instructed to be output was outputted in the past, accounting slip printing data generated from information in the accounting journal slip information while adding information showing the re-output thereto; and an output history management means 25 for writing the history of output of the accounting slip instructed to be output to the output history information in the storage means.
