Complexity associated with the application of the new Aged Care Funding Instrument (ACFI) has resulted in significant funding risk to Residential Aged Care Operators and as a result reduced quality of care outcomes for residents requiring complex health procedures for complex pain management intervention and palliative care. Optimisation of Care in line with the intent of the new ACFI has been far from realised by operators today as they struggle to apply sufficient process to optimise claiming and strengthen care provision particularly complex health care complex pain management intervention and palliative care. Although, complexity of the tool itself is a key reason why its application has been difficult, internal capacity of the organisation to coordinate the delivery of pain management and palliative care intervention can lack the sophistication needed to optimise results, as much in terms of quality care as in financial return. The One Care Network ACFI For Care Model has been developed as an innovative service delivery model that brings together unique Information Technology creations within a communication and organizational structure to improve the internal capacity of a residential aged care facility to support the delivery of such important aspects of service delivery today. Supported by innovative technology, a comprehensive project management structure, contemporary clinical management policies, communication and reporting systems, and a resource bank that supports Iocalised needs, the One Care Network ACFI For Care Model ensures quality service provision for older people in the residential aged care home in line with the needs of consumers today whilst maximising funding capability in the areas of Palliative Care and Complex Pain Management.
